IMPORTANT: Online registration and waiver submission are required. A separate medical form must be completed and signed in person at the gym before participation in any Summer Knights activities.

Participant Release and Hold Harmless Agreement

Please read the agreement below, then sign.

PARTICIPANT RELEASE AND HOLD HARMLESS AGREEMENT

I understand, acknowledge and agree that any and all of the activities in which I participate relative to the Hartford Knights’ Summer Camp (the “Summer Knights”) will be at my own risk and that I do hereby release the City of Hartford (the “City”), its elected and appointed officials, Hartford Knights (“HKYO”) and the HKYO Community Partner providing the Program (“Community Partner”), and the employees, agents, servants, officers, officials, volunteers and others working on behalf of the City, HKYO or the Community Partner (collectively “Program Providers”) from any and all claims, demands, liens, judgments, verdicts, actions, proceedings, damages, lawsuits, losses, awards and/or costs connected therewith, including but not limited to any and all damages that may be asserted, claimed or recovered against or from the Program Providers, whether individually or collectively, by reason of personal injury, including bodily injury or death and/or property damage, including loss of use thereof, arising out of or in any way connected or associated with my participation in the aforementioned activity and/or event.


This release and hold harmless applies to all risks arising out of or in any way connected or associated with said activity and/or event whether foreseen or unforeseen. If applicable, I do understand that a reasonable effort will be made to inform me of the general hazards involved with said activity and/or event. This release applies to damages suffered by me, by myself, as well as my family, my heirs, my successors, my assigns and any other party that might seek to make a claim through me, as a result of any harm or injury which I may suffer.


I agree to hold the Program Providers, individually and collectively, harmless from and against any and all claims made by myself, my family, my estate(s), my heirs, my successors or my assigns, arising out of or in any way connected or associated with the aforementioned activity and/or event.


I further agree that I shall hold harmless, indemnify and defend the Program Providers, individually and collectively, from and against any and all damages to persons or property, resulting from my intentional acts, provided, however, that I shall not be held responsible for and shall not indemnify the Program Providers, individually or collectively, from and against any gross negligence or willful acts of the Program Providers, whether individually or collectively.


I further assume the responsibility of the physical fitness and ability to participate in the aforementioned activity and/or event. If I do not feel I am capable of participating in said activity and/or event, I shall assume the responsibility of informing the appropriate person(s) in a timely manner.


In addition to the foregoing, I understand, acknowledge and agree that all photos, films, recordings and/or videos taken and/or made by a Program Provider in connection with my participation in the activity, are the property of the City, HKYO and the Community Partner and may be used in any or all City, HKYO or Community Partner publications, whether electronic or otherwise, and for other City, HKYO or Community Partner purposes.


FOR MINORS (required for participants under the age of 18): By signing below as parent or legal guardian, I certify that I have legal responsibility for the above participant. I have read and understand the significance of this release and hold harmless agreement and do consent and agree to his/her waiver, release, hold harmless, indemnification, defense and assumption of the risk as provided above.


3 on 3 - Registration

This is the registration SPECIFICALLY for the 3 on 3 tournament. We accept ages (9 – 19), third grade through high school.

Terms and Conditions

Participation in the 3 on 3 tournament is optional, but registration is required to participate in the Summer Knights-Open Gym Program. Students are expected to demonstrate excellent behavior throughout the program and failure to do so may result in removal from the tournament and gym.
